For Providers

Please fax referrals to the numbers below. Based on information provided, referrals will be triaged accordingly and appointment details faxed back to your office.

For gynecology referrals, please attach relevant bloodwork and imaging reports. 

For obstetrical referrals, please provide LMP and EDD, and attach if available any ultrasound reports, public health bloodwork, prenatal bloodwork, genetic screening results. Please inform patients that they will be delivering at Joseph Brant Hospital.
